We now accept referrals via Right to Choose referral to assess children between the ages 6-18 for Autism Spectrum Disorder (ASD) and Attention Deficit Hyperactivity Disorder (ADHD). Please note, referrals can only be accepted by Essex based GP referrals.

 

We understand that there is a lot of information out there and it may be hard to know what assessment might be the most beneficial, but we have tried to help with this!

To refer a child for an assessment to our service under Right to Choose NHS scheme please follow these steps:

 

1. Complete the referral form (by Parents and School) and take it to your GP

 

When you are ready to go ahead, can parents please download a copy of the document below and complete this with the help of the child’s school. Once complete please take it to your GP.

2. Ask your GP to refer you to Mina Medical Ltd under the category 'Children and Adolescent Service' within the eRS referral portal with the necessary referral forms.

 

When we receive the referral form we will contact you to let you know if we need any further information. After we have received all the information required we will contact you to book in your child’s assessment.
